We have an exciting opportunity for a motivated, dynamic, and results-driven individual.

We are looking for someone capable and proactive in leading and coordinating an integral part of our service, within our Severe Multiple Disadvantage (SMD) Team.

The role will support a team of substance use professionals employed by Change Grow Live, and embedded within Nottinghamshire Healthcare Trust. We recognise that people often experience multiple challenges in their lives which requires a seamless joint working approach.

Excellent communication and a flexible approach between services is essential in supporting service users to reach their goals.
