    Skills

    Hertfordshire CNS provides community support to adults of working age who are recovering from mental health issues, and additional complex needs including drug and alcohol misuse, housing issues, and learning disability.

    Responsibilities

    he key accountabilities of the role are:


    ü Developing, in consultation with Service Users, flexible and realistic support packages/person centred plans within agreed guidelines or service models.

    ü Ensuring that a collaborative approach is used, with effective communication links with external professional groups e.g. GPs, Social Services, etc. and to work as an effective member of any multi-disciplinary team.

    ü Ensuring record keeping is maintained to the required standard at all times and contributing to service monitoring requirements.

    ü Undertaking responsibility for clinical risk and needs assessment and the formation and implementation of risk management plans.

    ü Assisting in the development and implementation of service record keeping, procedures and policies.

    Experience Requirements

    About You:

    Essential:

    ü 6-12 Months proven experience in a similar role along with a good awareness of mental health problems or completion of a relevant degree.

    ü A Full UK Driving License and access to a vehicle.

    ü Flexible, patient and non-Judgemental.

    ü Good IT and Communication skills.
